Complete Midwifery Care Overview
Initial Interview
This is a complimentary initial consultation that lasts approximately 1-2 hours. Information about my practice, diet, nutrition and exercise is included. Common physical and emotional adjustments associated with pregnancy can be discussed at this tim. In addition, a medical history is taken.
Prenatal Care
Prenatal visits are scheduled regularly and will take place either in my home or yours. The visits are intended to be an informative, collaborative and fulfilling experience designed to meet the unique physical, intellectual, emotional and spiritual needs of the woman/couple. My typical prenatal care schedule for a low-risk woman with a normally progressing pregnancy is outlined below:
Weeks 4-28
1 visit per month (every 4 weeks)
Weeks 28-36
2 visits per month (every 2 weeks)
Weeks 36-Birth
1 visit per week
During the prenatal visits, we discuss your pregnancy and preparations for birth. Issues relevant to each particular stage of pregnancy are also discussed. Each visit will include checking your weight, pulse, blood pressure and urine. I will measure your fundal height to assess how baby is growing, listen to fetal heart tones and palpate your uterus to check baby’s size and position.
Lab Work
Routine prenatal lab tests, including blood type, Rh facor, rubella titer, serology, hepatitis screen and HIV may be drawn. I do not recommend the use of routine prenatal testing such as ultrasound or amniocentesis without a legitimate reason for doing so. However, I follow the recommendations for standardized maternal tests as set forth below:
Blood Panel
A complete blood count will be taken initially. This test includes the following: hemoglobin, hematocrit, analysis of red blood cells, white blood cell count, platelet count, a rubella titre, blood type, Rh factor, serology for syphilis, and hepatitis B screen. HIV testing is offered as well.
Repeat Hemoglobin & Hematocrit
These are taken at 28 and 36 weeks for all clients or as indicated.
Repeat Antibody Titre
This is the Indirect Coombs test and is done at 28 and 36 weeks for all clients with Rh- blood type.
Glucose Tolerance Test
This is a one-hour test with a minimum of a 50 gram glucose loading dose and is offered to clients between 26-28 weeks. Alternatively, a postprandial test via capillary draw is offered following a high-carbohydrate meal.
GBS
This is done between 28 and 36 weeks and is routinely offered to all clients.
Urinalysis
This is done routinely at all prenatal visits and checks the urine for protein, glucose, nitrites, ketones, bilirubin, leukocytes and blood.
PKU (Newborn Screening)
If parents choose to have the PKU test, I will arrange to do this at your home within 3 days of the birth.
PAP Smear
I offer this to all of my clients at the six week check-up and every two years thereafter as requested.
Home Visit
At least one home visit will be made during the third trimester, to assure that the home is ready for the birth and necessary supplies have been obtained.
